2009/2010 SUMMARY OF BENEFITS


This brief summary provides an overview of GWA Innovative Technology, Inc.'s benefits package.The information contained in this summary is subject to change and is not meant to be an official document.

 

o Medical Plan Summary

GWA Innovative Technology, Inc. (GWA) currently offers a Choice Plus PPO Plan. The PPO Plan gives an employee and his/her dependents the flexibility to choose an in-network or out-of-network provider.

 

o Prescription Drug Summary

The Prescription Drug Plan is available as part of your Medical coverage. Employees pay the Plan's co-payment to obtain a prescription from any participating pharmacy or through a mail-order benefit.

 

o Dental Plan Summary

The Dental Plan provides coverage for preventive and diagnostic services, restorative, and prosthetic treatment to help maintain healthy teeth. The Plan pays a percentage of the reasonable and customary cost of the services.

 

o Vision Plan Summary

The Vision Plan offers employees and covered family members the opportunity to receive various professional vision care services such as eye exams, glasses or contact lenses, and a Lasik discount.

 

o Flexible Spending Accounts

Two flexible spending accounts are offered which give employees the opportunity to put aside pre-tax dollars to pay for eligible health and dependent care expenses. A Flex Debit Card is available to employees at no charge.

 

o 401 (k) Savings and Retirement Plan

Employees are eligible to participate in the 401(k) Plan 1st of the month following the Date of Hire. Employees may contribute up to 80% of pretax earnings (subject to annual IRS limits). GWA matches a portion of the employee contribution.

 

o Voluntary Life Insurance

Employees may elect Supplemental Life Insurance coverage for themselves, spouses and children.

o Basic Life Insurance / Accidental Death and Dismemberment Insurance / Business Travel Accident Insurance
o Vacation / Personal Time / Holidays/ Bereavement Leave/ Jury Duty

 

o Employee Assistance Program (EAP)

The EAP is a free, confidential, counseling and advising service designed to help employees and family members deal with personal problems, legal/financial difficulties, family issues, drug/alcohol abuse, job/career issues, and/or emotional stress. The program also offers referrals for education, legal services, and financial planning.

 

o Fitness Assistance Program

Employees who attend bona fide fitness facilities are eligible to receive up to $250 a year for fitness membership expenses.

 

o Tuition Assistance Program

Tuition reimbursement is available for job-related continuing education at an approved institution that is generally recognized to be in good standing. Employees are responsible for any tax liability imposed by the IRS.

 

o Employee Referral Program

Employees are compensated for referring an individual for a GWA position upon the hiring and continuous employment of said individual.
